How to Arrange a Home Care Package Assessment
If you believe you or your loved one meet the eligibility criteria, you can initiate the assessment process by contacting My Aged Care, the Australian Government’s starting point for aged care services. You can reach them by phone at 1800 200 422 or through their website at www.myagedcare.gov.au.
During your initial contact with My Aged Care, you will be asked a series of questions to determine your level of need and whether a home care package assessment is appropriate. If deemed necessary, they will arrange for an ACAT team to conduct the assessment at your home.
