Provided you are over the age of 65 (or 50, if an Aboriginal and Torres Strait Islander person) or a person over the age of 50 at risk of, or experiencing, homelessness, you can undertake an assessment of your needs via the Single Assessment System. This will establish the types of care and services you may be eligible for and your priority level for receiving services.

If you are found eligible, the assessor will work with you to develop an individual support plan, providing the basis for you to understand your funding and the services you can receive.

You will receive a notice of decision with your support plan to share with your Support at Home provider. This will include the funding budget you have been approved for and a summary of your care needs and goals.

Once you know you’ve been approved for funding, you can choose your provider. Providers are registered according to the services they provide, so you can make sure you choose one that provides the services you need and are funded for. You will need to sign a Service Agreement with your chosen provider.

Once your funding becomes available, you’ll be assigned your budget and can begin receiving your services.
