Our team of experienced and qualified nurses and allied health professionals are here to support you so that you can live well at home.
We offer a broad range of clinical support services that are all covered under the Support at Home program – from wound care, physiotherapy, podiatry and much more.
Our multidisciplinary health team are passionate about delivering quality and safe care to older Australians. With decades of experience in aged care, our team are trained in the use of evidence-based prevention, diagnosis, treatment and management practices.
To maximise the potential of your Support at Home budget, your Care Partner will work with you to develop your personal Care Plan according to your preferences and needs.
Services offered under Clinical Supports
Nursing care
Includes a range of nursing care from registered nurses, enrolled nurses and nursing assistants.
Nutrition
Prescribed supplementary dietary products and aids for conditions related to functional decline or impairment.
Care management
Includes services that contribute to the overall wellbeing of an older person (e.g., care planning; service coordination etc).
Allied health and other therapeutic services
Includes a range of allied health and therapeutic services such as physiotherapy and occupational therapy.
Restorative care management
Includes specialist coordination services for older people undergoing the time limited Restorative Care Pathway.
What are the service costs?
Under the Support at Home Program, services in the clinical supports category are fully funded by government for all participants.
This means that participants will not be required to make any contributions to fees for clinical support.

Government support and funding
My Aged Care aims to make it easier for older people, their families, and carers to find information on ageing and aged care in Australia. My Aged Care provides online information and trained call centre staff to help get an older person's needs assessed and to find and access services.
