Under Support at Home, the End-of-Life Pathway provides additional support to help people remain at home if they are assessed as having 3 months or less to live.
The End-of-Life Pathway provides more funding for home care services: it provides the highest funding classification (per day) for in-home aged care services.
What are the service costs?
Under the Support at Home Program, additional funding for short-term supports is available to eligible participants.
These short term supports have individual budgets and timeframes, as per the details provided in the table below.

| Restorative Care Pathway | Provides a unit of funding of up to $6,000 (16 weeks). Can access a maximum of 2 units of funding over a 12-month period. |
| End-of-Life Pathway | $25,000 per eligible participant (services are accessible for up to 16 weeks). |
| Assistive Technology and Home Modifications | Based on the assessment you may be eligible for low ($500), medium ($2,000) and high ($15,000*) funding tiers. (Services are accessible for up to 12 months.) |
