Care Management
Under Support at Home, care management is designed to optimise your safety, health, wellbeing and quality of life. Care management applies to all participants receiving services under all Support at Home classifications.
Led by experienced nurses, our team can support you throughout every step of your in-home care journey.
Care management services are extensive and include:
- Care planning
- Service planning and coordination
- Development, review and evaluation of a participant’s care plan and quarterly budget
- Monitoring of services provided
- Provision of information and education about the participant’s care and services
- Support to connect to health and other services when needed
- Support to undergo a Support Plan Review, if needed
Support at Home participants, who receive ongoing services, will have 10% deducted from their quarterly budget for care management.
What are the service costs?
Under the Support at Home Program, services in the clinical supports category will continue to be fully funded by government for all participants.
This means these services are fully funded by government and participants will not be required to make any contributions to fees.

Nursing care
Includes a range of nursing care from registered nurses, enrolled nurses and nursing assistants.
Nutrition
Prescribed supplementary dietary products and aids for conditions related to functional decline or impairment.
Care management
Includes services that contribute to the overall wellbeing of an older person (e.g., care planning; service coordination etc).
Allied health and other therapeutic services
Includes a range of allied health and therapeutic services such as physiotherapy and occupational therapy.
Restorative care management
Includes specialist coordination services for older people undergoing the time limited Restorative Care Pathway.
Government support and funding
My Aged Care aims to make it easier for older people, their families, and carers to find information on ageing and aged care in Australia. My Aged Care provides online information and trained call centre staff to help get an older person's needs assessed and to find and access services.
